I had a similar problem. I suspected it being worn cogs when I replaced
the chain, but it turned out to be a couple of slightly bent teeth on
the chain ring. I straightened them out with some piers (there's
probably a better way to do it) and it was then as good as new. I'm
still not sure how they got bent. My suspicions are that they got bent
when the bike was folded in the trunk of the car.
Check for bent teeth before replacing the cassette (or chain). Take the
chain of the chain ring, spin the crank and look to see if any of the
teeth look out of alignment with the others. Mine were only slightly
bent and it was hard to see when the crank wasn't turning.
