Thanks for sharing Tony. That is quite interesting. Although, it may be the case that aspects associated with facilities might be implicit in the ratings. Anyway, I am in the camp that finds it difficult to believe that global facilities are needed -- or would produce -- a "good" (whatever that means) cycling environment. However, I think that is hard to argue against the statement that roads could be better constructed for better interaction between drivers and cyclists.

The Dutch have set up an audit called Cycle Balance that audits their
towns and cities for cycling conditions. Bicycle facilities, other than
bicycle parking, do not feature in the measures they use for the audit.
According to the leader of the Cycle Balance audit "How many cycle
paths or lanes a town has is not important. A town with very few cycle
lanes and paths can be a very nice and safe town to cycle around in and
a town with lots of cycle lanes and paths can be horrible and dangerous."
If Prof Pucher is right he needs to explain why the Dutch audits ignore
the factor he seems to consider an essential requirement
