[Yak] Trailer Danger

On a recent tour the tow arm of the trailer behind our tandem traveller detached from the trailer. This occurred at low speed in a suburban setting with no cars around. The bolt connecting the arm to the trailer had worked loose over a two  week ride and we were perfectly unaware this could /would happen. Luckily it was not a high speed descent! Luckier still I could retrace my steps and I found bolt and washer and was able to get going again.  Has anyone else encountered this?  Has anyone got a suggestion as to a locking nut to prevent this from happening. I now check each bolt each day !
